Trever Wagner, PT, DPT, ATC
Physical Therapy and Sports Rehabilitation Provider in Sioux Falls
Trever Wagner is a Doctor of Physical Therapy and Certified Athletic Trainer at Vitalis MVMT + Wellness in Sioux Falls. With nearly ten years of clinical experience, Trever helps patients reduce pain, restore mobility, and rebuild strength so they can get back to the activities they love.
Trever’s passion for physical therapy is personal. Having experienced injuries himself, he understands firsthand how pain and functional limitations can affect daily life, mood, and confidence. That experience fuels his commitment to evidence-based care, patient education, and a treatment plan that fits the whole person, not just a sore joint.
Clinical Philosophy
Whole-body evaluation, individualized plans, and education that builds confidence
Trever’s treatment philosophy starts with the full picture. He incorporates what a patient tells him, what his testing and measurements reveal, and what current research supports to create an individualized plan that targets the true cause of pain and dysfunction.
A core part of his approach is education. Trever often meets patients who are skeptical of physical therapy because of a past experience where they didn’t feel heard or didn’t understand the “why” behind the plan. His priority is to change that experience by building trust from the very first visit through clear explanations, safe progression, and a plan that makes sense.
He also emphasizes something many patients underestimate: healing takes time. While the goal is always to reduce pain efficiently, long-term recovery typically involves a process of improving mobility and flexibility, then building strength and stability to prevent flare ups and recurring issues.
What Makes Trever’s Approach Different in Sioux Falls
Trever treats beyond the painful spot. He evaluates the body as a connected system because pain in one area is often influenced by movement patterns and weaknesses elsewhere. A sore knee might be connected to hip strength. A recurring ankle sprain may trace back to stability deficits, flexibility limitations, or poor mechanics.
He also uses a broad “toolbox” of manual therapy techniques, therapeutic tools, and exercise progressions rather than relying on one approach. If something isn’t working, he can adjust quickly and strategically—often accelerating results and improving long-term outcomes.
Areas of Specialization
Trever is especially skilled at treating:
- Sports-related injuries and return-to-sport rehab
Years of experience, plus continuing education focused on athletic injuries, allow Trever to guide athletes from pain to performance with a plan that prioritizes strength, mechanics, and re-injury prevention. - TMJ dysfunction and migraine patterns
Trever’s combination of education, hands-on care, functional dry needling, and exercise-based interventions has helped many patients reduce the frequency and severity of headaches and jaw-related symptoms. - Chronic pain and stubborn conditions
When pain has lasted long enough to change how someone moves, sleeps, and functions, Trever builds a step-by-step plan to restore motion, calm irritation, and rebuild stability—so the issue is less likely to return.
How Trever Evaluates Complex Symptoms
Comprehensive testing and pattern recognition
Trever’s treatment decisions are guided by a thorough initial evaluation that may include range of motion and strength testing, special orthopedic tests, movement assessment, and an in-depth history. His ability to shift interventions based on how the patient responds helps him find the most effective path quickly.
He also watches for red flags that suggest symptoms may be more than “just pain” or “just aging,” including night pain, rapid weight changes, relevant family history, or abnormal tissue findings. When appropriate, he refers patients out for further evaluation or imaging.
Trever may recommend imaging or additional medical support when progress stalls—often after a focused period of care. He is thoughtful about injection pathways and tends to avoid recommendations that may lead to long-term issues. His goal is always to choose the option that best supports durable recovery.
Functional Dry Needling
A targeted technique used to reduce muscle tension, calm irritated trigger points, and improve mobility. Trever explains what patients may feel during and after treatment so they understand the process and feel confident throughout care.
Manual Therapy and Soft-Tissue Techniques
Hands-on approaches to improve joint mobility, tissue flexibility, and movement quality. Trever uses different techniques depending on the condition and the patient’s tolerance and goals.
Instrument Assisted Soft Tissue Mobilization
A tool-assisted approach used to address tissue restrictions and improve mobility, often helpful for overuse injuries and chronic tightness patterns.
Cupping Therapy
A soft-tissue technique that may support circulation and mobility, commonly used as part of a broader recovery plan.
RockTape and Athletic Taping
Supportive taping strategies that can improve comfort, reduce irritation, and enhance movement confidence during activity.
Vertigo and Balance Treatment
Assessment and care for dizziness and balance-related symptoms with the goal of improving stability, confidence, and daily function.
TMJ Dysfunction Treatment
Focused support for jaw pain, tightness, clicking, and related headache patterns through targeted mobility work, soft-tissue techniques, and movement strategies.
Cervical and Lumbar Decompression Techniques
Gentle strategies designed to reduce irritation, improve mobility, and relieve pressure-related symptoms in the neck and low back.
Post-Surgical Rehabilitation
Guided recovery plans to restore strength, range of motion, and function after surgery with safe progression and measurable improvements.
Acute and Chronic Joint or Muscle Pain Care
Evidence-based plans that target the cause of pain, reduce flare ups, and improve strength and stability for long-term results.
Patient Outcomes and Success Stories
Trever is motivated by life-changing results, especially for patients who have struggled for years.
One example is a patient who experienced chronic migraines since her teens. Through a combination of education, functional dry needling, and targeted interventions, her migraines became far less frequent and significantly more manageable.
Another standout transformation involved a runner with Achilles tendon pain that prevented him from doing what he loved. With Winback therapy, functional dry needling, and progressive exercise-based rehab, his symptoms improved quickly, and he returned to running hills and trails within about six weeks.
What Success Looks Like Long Term
For many acute and chronic pain cases, Trever aims for improvement within a typical plan of care—often 4–12 weeks—so patients can return to life without feeling dependent on ongoing visits. For post-surgical cases, longer care timelines may be appropriate.
Long-term success means:
- The patient understands what caused the issue
- Pain stays controlled through tools learned in therapy
- Strength and stability prevent future flare ups
- The patient is back in the community doing what they love
Trever measures progress using objective and functional benchmarks such as range of motion, strength, reflexes, special tests, and functional scales, alongside real-world improvements like returning to running, lifting, working, or playing sports without pain.
Trust, Communication, and Patient Experience
Trever believes the first visit is crucial. His goal is for every patient to feel welcomed, heard, and confident that they are receiving high-quality care.
He explains complex concepts in plain language, uses demonstration to teach movement patterns, and often uses mirrors during exercises so patients can see compensation patterns and improve technique in real time.
He also emphasizes commitment. Home exercises and lifestyle changes are not “busywork”—they are part of what makes results last.
Integrative Care at Vitalis MVMT + Wellness
Trever values collaboration when it improves patient outcomes. Vitalis offers multiple wellness services under one roof, and Trever may coordinate care with Ashley Wagner, CNP, or other trusted providers when inflammation, recovery, or lifestyle support could improve results.
This team-based approach allows Vitalis to support the whole person—combining movement-focused rehab with broader wellness tools when appropriate.
Personal Story and Values
Trever was born and raised in Mitchell, SD. He earned his bachelor’s degree in Athletic Training from Dakota Wesleyan University, then completed his Doctorate of Physical Therapy degree at the University of Mary in Bismarck, ND.
As a parent, Trever approaches patients the way he would want his own children to be treated—with attention, compassion, and a family-first mindset. He describes the Vitalis environment as friendly, laid back, and welcoming, because patients should never feel intimidated when they’re seeking help.
Outside the clinic, Trever enjoys spending time with his wife and kids, working out, and watching sports. His day-to-day commitment to movement and fitness shapes how he coaches patients: movement matters, recovery matters, and consistency matters.
Credentials and Advanced Training
Trever is proud of earning his doctorate degree and of becoming certified in functional dry needling—an intervention he believes has significantly improved his ability to help patients get relief and regain function.
He is also the only clinic in South Dakota to offer Winback therapy, a form of TECAR therapy, which expands the treatment options available for recovery and pain reduction through cellular healing.
Trever stays current through continuing education, ongoing research, and condition-specific study so his interventions reflect the latest evidence and best practices.
